      FMS (fibromyalgia syndrome) is a widespread musculoskeletal pain and fatigue disorder for which the cause is still unknown. Fibromyalgia means pain in the muscles, ligaments, and tendons – the soft fibrous tissues in the body.

      Most patients with FMS say that they ache all over. Their muscles may feel like they have been pulled or overworked. Sometimes the muscles twitch and at other times they burn. More women than men are afflicted with FMS, and it shows up in people of all ages.

      To help your family and friends relate to your condition, have them think back to the last time they had a bad flu. Every muscle in their body shouted out in pain. In addition, they felt devoid of energy as though someone had unplugged their power supply. While the severity of symptoms fluctuate from person to person, FMS may resemble a post-viral state. This similarity is the reason experts in the field of FMS and chronic fatigue syndrome (CFS) believe that these two syndromes may be one and the same. Gulf War syndrome also overlaps with FMS/CFS.


      Pain – The pain of FMS has no boundaries. People describe the pain as deep muscular aching, throbbing, shooting, and stabbing. Intense burning may also be present. Quite often, the pain and stiffness are worse in the morning and you may hurt more in muscle groups that are used repetitively.

      Fatigue – This symptom can be mild in some patients and yet incapacitating in others. The fatigue has been described as “brain fatigue” in which patients feel totally drained of energy. Many patients depict this situation by saying that they feel as though their arms and legs are tied to concrete blocks, and they have difficulty concentrating, e.g., brain fog.

      Sleep disorder – Most FMS patients have an associated sleep disorder called the alpha-EEG anomaly. This condition was uncovered in a sleep lab with the aid of a machine which recorded the brain waves of patients during sleep. Researchers found that most FMS patients could fall asleep without much trouble, but their deep level (or stage 4) sleep was constantly interrupted by bursts of awake-like brain activity. Patients appeared to spend the night with one foot in sleep and the other one out of it.

      Sleep lab tests may not be necessary to determine if you have disturbed sleep. If you wake up feeling as though you’ve just been run over by a Mack truck – what doctors refer to as unrefreshing sleep – it is reasonable for your physician to assume that you have a sleep disorder. Many FMS patients have been found to have other sleep disorders in addition to the alpha-EEG, such as sleep apnea (as well as the newly discovered form of interrupted breathing called upper airway resistance syndrome, or UARS), bruxism (teeth grinding), periodic limb movement during sleep (jerking of arms and legs), and restless legs syndrome (difficulty sitting still in the evenings).

      Irritable Bowel Syndrome – Constipation, diarrhea, frequent abdominal pain, abdominal gas, and nausea represent symptoms frequently found in roughly 40 to 70% of FMS patients. Acid reflux or gastroesophogeal reflux disease (GRED) also occurs with the same high frequency.

      Chronic headaches – Recurrent migraine or tension-type headaches are seen in about 70% of FMS patients and can pose a major problem in coping for this patient group.

      Temporomandibular Joint Dysfunction Syndrome – This syndrome, sometimes referred to as TMJ or TMD, causes tremendous jaw-related face and head pain in one quarter of FMS patients. However, a 1997 published report indicated that close to 75% of FMS patients have a varying degree of jaw discomfort. Typically, the problems are related to the muscles and ligaments surrounding the jaw joint and not necessarily the joint itself.

      Other common symptoms – Premenstrual syndrome and painful periods, chest pain, morning stiffness, cognitive or memory impairment, numbness and tingling sensations, muscle twitching, irritable bladder, the feeling of swollen extremities, skin sensitivities, dry eyes and mouth, dizziness, and impaired coordination can occur. Patients are often sensitive to odors, loud noises, bright lights, and sometimes even the medications that they are prescribed.

      Aggravating factors – Changes in weather, cold or drafty environments, infections, allergies, hormonal fluctuations (premenstrual and menopausal states), stress, depression, anxiety and over-exertion may all contribute to symptom flare-ups.


      The cause of FMS remains elusive, but there are many triggering events thought to precipitate its onset. A few examples would be an infection (viral or bacterial), an automobile accident or the development of another disorder, such as rheumatoid arthritis, lupus, or hypothyroidism. These triggering events probably don’t cause FMS, but rather, they may awaken an underlying physiological abnormality that is already present.

      What could this abnormality be? Theories pertaining to alterations in pain-related chemical transmitters (particularly substance P, nerve growth factor, serotonin, and norepinephrine), immune system function (e.g. abnormally elevated levels of cytokines that form the communications link between your immunologic and neurologic systems), sleep physiology, and hormonal irregularities are under investigation. In addition, modern brain imaging techniques are being used to explore various aspects of brain function. The body’s response to exercise, stress, and alterations in the operation of your autonomic nervous system (the one that operates in your peripheral tissues) are also being evaluated. Substance P and nerve growth factor are increased threefold and fourfold (respectively) in the spinal fluid of people with FMS, but researchers are working to figure out why these elevations exist. With regards to genetics, its role in FMS is also the focus of many investigations.


      Traditional treatments are geared toward improving the quality of sleep and reducing pain, which means that a sleep study may aid with individualizing your therapy. Deep level (stage 4) sleep is crucial for many body functions (such as tissue repair, antibody production, and the regulation of various neurotransmitters, hormones and immune system chemicals). Therefore, the sleep disorders that frequently occur in FMS patients are treated first because they may be a strong contributing factor to the symptoms of this condition. Medications that boost your body’s level of serotonin and norepinephrine (neurotransmitters that modulate sleep, pain, and immune system function) are commonly prescribed in low doses, such as amitriptyline, cyclobenzaprine and Cymbalta. Ambien, Lunesta, clonazepam, and trazodone are just a few of the medications that may be used to aid sleep. Ultram may help with the pain, although stronger opioids may be needed for treating moderate to severe pain. Muscle relaxants, anti-epileptics (such as Neurontin and Lyrica) and other drug categories may be prescribed as well.     What is the difference between Osteopenia, Osteoporosis, and Osteomalasia?
    Ok so I think osteoporosis I and II with age, osteomalasia is like rickets in adults, due to nutrition like D and calcium. Then what is the difference between osteopenia and osteoporosis?

      Osteopenia is simply what occurs before Osteoporosis.

      Basically our bones have a mineral density — which peaks around age 30 to 35.

      When our bone mineral density (bone density) is lower than average for your ‘age’ but not severely low, it is termed osteopenia. Generally osteopenia is completely reversible .. especially in those under 55 with nutritional protocols, exercise, and so forth.

      Osteoporosis , likewise, is just an advanced more severe form of bone mineral density lowering. It is Osteopenia but more significant.

      Sort of like if your bone density is 20 normally, with osteopenia it would be 15, with osteoporosis it would be 10 (or lower).

      Osteopenia is treated more with lifestyle and supplements, Osteoporosis is treated the same way, but some designer drugs are utilized to treat the more severe ailment.

      Osteomalacia is what you considered — It is sort of ‘rickets’ in adults, but without the deformation. It is specifically a Vitamin D deficiency in adults, that results in bone deformation/density issues. Which results in pain, fragile bones, and other problems. This is different from the other two because it is ALWAYS a deficiency with vitamin D… either in the amount in the diet, or an absorption problem with the person.

      Osteomalacia generally produces more symptoms that the other two — as it commonly causes pain, that is difficult to diagnose.. and only goes away with resolution of the vitamin D deficiency (which can take 6months to a year).

      Ie: I’ve had patients with muscle, joint and bone pain that had , at best, a Dx of fibromyalgia — when tested, their vitamin D levels were under 20 out of a reference range of 35-100. Usually below 15. This is severely deficient, and many had been deficient, likely, for many years.

      After treatment and having their levels return to a normal range, they still experienced many symptoms even 3-4 months after treatment — but with consistency of treatment I found their main complaints of pain and weakness resolved within 6months to a year. Apparently the body has a great deal of ‘rebuilding’ to do before these types of symptoms resolve in something like Osteomalacia.

      Though keep in mind there is not always pain or any symptoms with any of these conditions — but they are fairly easy to diagnose if the right tests are performed. Generally a Bone density scan will detect the first two, and sometimes Osteomalacia — combining the scan with a Vit. D test will nearly always detect Osteomalacia and even if the individual is in normal range, but on the ‘low’ end I will still Dx Osteomalacia if they exhibit the symptoms that correspond with it. IE: D level of 35-40, and a bit less density than I would like to see for their age. I’ve seen 30yr olds with the density of 60yr olds. The good side is the 30yr old can rebuild that bone quickly and usually suffer no permanent effects due to their age.

    How do you know when medication is necessary?
    I think a lot of people are on meds that don’t necessarily need them, but doctors seem so quick to prescribe them. I really don’t like the idea of meds, but I am so depressed and suicidal. I am socially anxious and I have a really low self esteem and anxiety. It’s not like I cry everyday or comtemplate suicide every night, but I still feel depressed 75% of the time. How do you know when you need meds?
    My therapist has suggested meds before but I dont know if I need it.

      Hi there, I go by Andy but I am a mother and grandmother. Yes you are right, thousands of people are on meds, but is that the way to go? There are some natural things to try first. Then if they don’t work, consider the other choice. I am an herbalist and I have been using herbs for over 30 years. I use St. Johns Wort for depression. It actually balances your moods so you feel better about your life and the day, it also keeps me from feeling tired all the time. Also if your diet is not right that will affect your health, which also means your mental health. Make sure you are getting the Omega Threes in your diet everyday as your brain will begin to show depression without it. If you eat out alot like at these fast food places then you are getting alot of Omega Six’s and they cannot dock in the brain to be utilized. So you begin to be depleted of it and when you do , depression sets in. Hope that helps. Diet is so important and getting enough rest is also! I take Omega Three’s up to three times a day but I am older. There is another product that is natural that you can try. It is called SAM (S-Adenosylemethionine. Normally the body manufactures all the SAM it needs from the amino acid methionine. However, a deficiency of methonine, vitamin B12 or folic acid can result in decreased SAM synthesis. In addition, tissue levels of SAM are typically low in the elderly and in patients suffering from osteoarthiritis, depression, and various liver disorders. There are five principal conditions where SAM is used: depression, osteoarthiritis, fibromyalgia, liver disorders and migraine headaches. Supplementing the diet with SAM
      in depressed patients results in increased levels of serotonin, dopamine, and phosphatidylserine. It imporves binding of neurotransmitters to receptor sites, which causes increased serotonin and dopamine activity and improved brain cell membrane fluidity, all resulting in significant clinical improvements. Based on clinical studies, it appears that SAM is perhaps the most effective natural antidepressant (although a strong argument could be made for the extract of St John’s Wort. Oral preparations at a dosage of 400 milligrams four times daily (1,600) milligrams total demonstrate that SAM is just as effective orally as it is intravenously. SAM is better tolerated and has a quicker onset of antidepressant action than tricyclic antidepressants. SAM produces significant effects in the postpartum (after pregnancy) period and drug rehabilitation, SAM’s benefits in these conditions probable stem from a combination of its effect on brain chemistry and liver function. In the study in postpartum depression (after-pregnancy “blues”) the administration of SAM (1,600) milligrams per day produced significantly better mood scores than a placebo group. As for the use of SAM in drug detoxification, SAM (1,200) milligrams daily significantly reduced psychological distress (chiefly anxiety and depression) in the detoxification and rehabilitation of opiate abusers.

    I’d like to try Lyrica for my Fibromyalgia, but causes weight gain, How can I avoid that? Medicals, please.?
    I have tried all meds for my FM, Lyrica seems to be best choice- but I do NOT want to gain any weight, can I avoid that side effect?

      Lyrica or pregabalin can cause weight gain in some of the patients, but not all patients experience weight gain, nor do all have the issues with the medication that one of the other responders reported. Incidentally, pregabalin is so named becuase it is a derivative of the GABA neurotransmitter, and has nothing to do with “stepping up” to gabapentin. In fact these medications appears to have different mechanisms of action. Also, you need to understand certain points about treatment of FMS with Lyrica. It does not (normally) completely relieve FMS pain, but is usually 50% effective. It usually takes a month or more to reach the optimum dose, and stabilize, and at that point you may also have any side effects tend to diminish some. I have been on Lyrica at 100 mg three times a day for 3 years for neuropathic pain, and have yet to have any significant change in my weight, except that I have lost some when trying to lose weight.

      All medications have side effects, and some patients experience the side effects to a greater extent, while others have few or no side effects on the same dose. Your health care providers have weighed the benefits of the medication against any potential for side effect.

      I urge you not to be overly influenced by the individual experiences of people who post their responses on YA. Health care professionals make their decisions based on carefully controlled studies in which the outside variables are better controlled, as opposed to individual anecdotal comments. I could report that I have anaphylactoid reactions to ibuprofen (which is true) but that does not change the fact that millions of people take ibuprofen without side effects. If a 110 year old man tells you he has smoked cigarettes and consumed a quart of whiskey everyday since he was 15, it does not make these healthy lifestyle choices.

      If when taking Lyrica, you notice weight gain, you should first look for signs of water retention. That is the most important issue which weight gain would be associated with. Otherwise, follow a healthy diet and get regular exercise. One factor which is often not considered when people note the weight gain after starting some medications for pain conditions, is that their lifestyle usually becomes more sedentary as the pain condition worsens to the point where prescription medications are required. This is not the only factor with Lyrica, but it is one of the contributing factors. If possible, get yourt physician to start with a low dose and increase at a slow rate. This won’t provide as much relief as quickly, but your body will adapt to the medication, and the side effets will be diminished.

    I have very severe fibromyalgia and the Dr. has me take 8 10/325mg tablets of Percocet everyday. I have been doing this now for a few years and I was wondering what the long term effects could be? Can it cause you to have heart problems? The reason I’m asking is because I have been having chest pains the last few weeks. I had a stress test done and it came out fine. However my cholesterol is through the roof. Could the pain be related to high cholesterol?

      Wow. Just – wow. Does anybody bother to research their answers?

      Percocet contains oxycodone (which is NOT oxycontin – it’s actually a drug that’s closely related to codeine) and acetaminophen (like Tylenol). Acetaminophen can be toxic to your LIVER at fairly low doses when mixed with as little as two alcoholic drinks.

      As for your dosage, 10/325 isn’t one of the pill strengths specifically listed in the information I could access easily, but it sounds fairly normal compared to the others. As long as you don’t take ANYTHING else that contains acetaminophen, and DO NOT drink alcohol, you should be reasonably safe.

      Many other medications contain acetaminophen, so before taking any kind of over-the-counter or prescription medication, you need to be absolutely sure that they don’t contain acetaminophen.

      As for acetaminophen causing heart problems or whatever, it’s actually the one OTC analgesic heart patients are allowed to take when they’re on blood-thinning medications. Ibuprofen and aspirin are off-limits! So you don’t need to worry there.

      Fibromyalgia DOES NOT cause any connective tissue changes, either. Nope, not any. It is a neurological disorder of hypersensitivity, with increasing amounts of evidence to prove that it’s based on a genetic weakness.

      On the other hand, a study that was released this week found that women with fibromyalgia are 5.56 times more likely than a control population to have “metabolic syndrome,” which puts them at greater risk for heart disease. That’s just one study, but there are others that indicate cardiovascular issues in patients with the most severe cases of fibromyalgia.

      High cholesterol is something you want to get under control, no matter what else is going on. It shouldn’t cause pain – but stress can lead to chest pains, and to increased risk of heart disease. Fibromyalgia will amplify any kind of pain you experience (that hypersensitivity), and stress causes fibromyalgia symptoms to intensify.

      So – reducing the stress in your life will help the fibromyalgia AND may make the chest pains go away. It certainly can’t hurt 🙂 One of the best ways to reduce your cholesterol levels is to watch what you’re eating. A balanced diet of whole, fresh foods, as close to their sources as you can manage (not junk, not packaged or processed) will serve you well both for controlling your cholesterol and improving your fibro symtpoms.     What is the element sulphur used for in naturopathic medicine?
    Could high levels of sulphur in your water effect you the same way as taking naturopathic remedy of sulphur? Thanks for all good answers. 🙂

    Is it safe to take malic acid if you take lexapro?
    I hear malic acid is good for fibromyalgia but am unsure if its ok to take with lexapro.

      According to Lexapro’s site:

      Can I take Lexapro with other medicines?

      Lexapro has a low risk of interacting with other medicines in general. One important exception is the family of antidepressants called monoamine oxidase inhibitors (MAOIs). Lexapro and MAOIs should not be taken together or within 14 days of each other. As with other medicines in this class (SSRIs), caution is indicated when taking Lexapro with tricyclic antidepressants (TCAs).

      As with other psychotropic drugs that interfere with serotonin reuptake, patients should be cautioned regarding the risk of bleeding associated with the concomitant use of Lexapro with NSAIDs, aspirin, or other drugs that affect coagulation. Before you begin taking Lexapro, make sure to tell your healthcare professional if you are taking any other medicines, including over-the-counter medicines, herbal remedies, diet supplements, etc.

    Fibromyalgia – I would like to hear how others are treating this condition?
    I’m pretty frustrated with my hmo. In order to get decent treatment, I’m having to go to a dr. outside of my hmo & pay out of pocket. I’m so frustrated with the constant tiredness and my flares last FOREVER. Any help would be appreciated!

      I have had fibro for about 8 years now. I take tramadol for pain, amitriptyline for sleep.

      I have changed my diet extensively avoiding all the following foods: Caffiene in any form (Yes colas and chocolate also) No carbonated drinks, no foods with MSG, no foods with aspartame family of sweetners, no food with added sugar , no preprocessed foods. Also in my case I have to give up gluten foods. I basically eat fresh fruits and vegies and lean meats. This has been difficult but it has made huge difference in my pain levels. Drinking lots of water can help also.

      I also take the following vitamins: multiple vitamin, vitamin c, b-complex, magnesium/calcium/ zinc blend.
      Finally the one supplement that has really helped lower my pain levels and boost my energy is DRibose. Its the D in your “DNA” . Its being used by many fibro doctors to treat their patients with marked success.

      I have also found that exercising in water is very helpful in reducing pain.

      As you know with fibromyalgia you will also have lots of other problems, treating all your problems together is a must for lowering pain levels. One source of increased pain in fibromylagia is food allergies or sensitivities. If you continue to eat foods your body is sensitive to you will continue to have increased muscle pain.

      One of the easiest ways to help yourself is to keep positive thoughts. When we have negative thoughts our brain sends out signals to increase certain chemicals in our bodies that can cause pain in our muscles. Its something we can control easily on our own. To stop that cascade of pain producing chemicals just change the way you think. Thats why meditation works so well for many people.
